Appointments
We are currently accepting new patients! Please give us a call at 541-673-0131, or fill out our online form to request an appointment. Once you make an appointment, we reserve that time for you. If for any reason you need to reschedule your appointment, please provide us with at least 48-hours’ notice.
What to Bring to Your First Appointment
Please bring the following items with you when you come for your initial visit:
- Insurance Information/Card
- Photo Identification
- Any medical records, including all medications and supplements
Please arrive 15 minutes before your scheduled appointment time to fill out necessary paperwork.
What to Expect During Your New Patient Exam
Your new patient exam with Dr. Alexis is an exciting opportunity for you to get a great start on your journey towards a healthier smile. During this appointment, Dr. Alexis will discuss with you both your dental and general health history. She will also ask you about any questions, concerns, or goals you may have about your smile. By performing a thorough exam, she will check for signs of developing cavities, gum disease, oral cancer, or other dental health issues. She will also take digital x-rays to gain a more comprehensive view of your mouth. Based on her findings, she will suggest a treatment plan that will help you achieve your healthiest, most beautiful smile.

Our fees reflect the skill, craftsmanship, and attention to detail required to provide custom dental solutions tailored to your unique needs. During your consultation, we will discuss costs and payment options to make your treatment affordable and accessible for you.
We accept cash, check, American Express, Discover, MasterCard, Visa, and debit. Third-party financing is available through CareCredit.
Insurance Information

No Dental Insurance? Ask About Our GEM Club!
Our GEM in-house membership plan serves patients without dental insurance. Through the program, you can receive preventive care, exams, and a reduced fee for restorations. We are members of several preferred providers including Delta Dental, Cigna, MetLife, Regence, Blue Cross Blue Shield, LifeMap, and Healthcare Management.
